The 82 units of supportive housing will be funded with New York State Empire State Supportive Housing Initiative (ESSHI) service dollars and will serve individuals with a serious mental illness (SMI) and chronically homeless individuals and families. Amenities include on site laundry facilities on each floor, exercise/wellness room, bike storage, electronic bike/scooter charging on property, energy efficient appliances and HVAC systems, community room and lobby, playground and community space outdoors, access to green space and 24/7 on site front desk staff.

Medford Gardens

A mixed-use apartment development in Medford, NY and will consist of one three-story building with 67 units of affordable and supportive housing and one superintendent unit. Town of Brookhaven zoning requires a 55+ age restriction for the property. The supportive units will be funded with New York State Empire State Supportive Housing Initiative (ESSHI) service dollars and will serve individuals who meet the ESSHI definition of homeless, who are over 55+ and have a serious mental illness (SMI) and for chronically homeless individuals. The building will include onsite laundry facilities on each floor, exercise/wellness room, energy efficient appliances and HVAC systems, community room and lobby, access to green space and 24/7 on site front desk staff.

Bishop Jones Community Residences

Bishop Jones Community Residences is the new construction of two three‐story buildings which will include affordable and supportive apartments. The supportive housing units will be funded by the Empire State Supportive Housing Initiative (ESSHI) and will serve individuals or families with a qualifying individual, who meet the ESSHI definition of homeless, and who are diagnosed with a substance use disorder (SUD). Each of the two buildings will have common spaces consisting of a building community room with serving kitchen, common corridors, building management offices, laundry facilities, trash & recycling rooms, and building services. Building services include main lobby & reception desk, manager’s office, program offices, maintenance room, water & electric service & equipment rooms. The project will increase the availability of affordable and supportive housing for individuals and families living in Syracuse, NY, especially the southside.

Playter Gardens at The Market

Playter Gardens is 73-units of affordable, integrated supportive housing located along all City of Buffalo vacant lots along Playter Street starting at 388 Paderewski Drive, Buffalo, NY 14212. There will be 37 units designated for a combination of aging out of foster care and at risk young adults ages 18-25 years old. All units will be affordable to households with income below 60% of Area Median Income and all supportive units will include rental assistance funded by the Empire State Supportive Housing Initiative (“ESSHI”). The development will include an open lobby and mail room inspired by trauma informed design principles but driven by a modern interior design precedent, a secure package drop, indoor bicycle and bulk locker storages, a community room with a business center / computer lab and gaming area for young adults and resident service offices with design to meet the needs of the service provider partner.

The Meadow on Seneca

The Meadow on Seneca will be type V new construction with 4 stories of residential over a concrete podium, located on the corner of N Meadow Street and W Seneca St in the City of Ithaca; the approximate address is 116 N Meadow St., Ithaca, NY 14850. The development will consist of 70 units and will target a mixed population of affordable housing with half of the units set aside for persons with serious mental illness and households experiencing chronic homelessness. All units will be affordable to households with income below 80% of Area Median Income and all supportive units will include rental assistance funded by the Empire State Supportive Housing Initiative (“ESSHI”). The building will include first floor office and amenity space, fitness room, rooftop terrace, resident services, on-site laundry, an open area main lobby with a greeting desk, utility rooms, a community room and one elevator.

Hooper Road Apartments

A single, four-story, 80-unit, senior mixed income affordable housing development. Hooper Road Apartments (“Project”) will be located at 609 Hooper Road in the Town of Union, Broome County, NY 13760. Forty of these will be for seniors, aged 55+. The remaining 40 units will be reserved for frail elderly individuals, aged 55+. The building will feature common access amenities such as a spacious lobby seating area, a community room with a kitchen, fitness room, laundry, and a free access computer area.

New construction of 66-units, multifamily, affordable and supportive apartments. There will be 40 units of supportive housing set aside for homeless adults with psychiatric disabilities and homeless veterans with psychiatric disabilities. All units will be affordable to households with income below 60% of Area Median Income. The building will include 42 studio apartments, 17 one-bedroom apartments, 6 two-bedroom apartments and 1 two-bedroom superintendent unit. The building will include an exercise room, computer room, community room, and laundry facilities.

The proposed development will convert an existing vacant former manufacturing warehouse into 109 new and high-quality affordable apartments for individuals and families who suffer from substance use challenges. This project will renovate three connected existing buildings into 81 units and build a three-story addition with an additional 27 units. Project amenities will also include a community room, exterior recreation space in the courtyard, and a fitness center. Lounges, laundry rooms, and trash rooms will be located on each floor. A community room and secure resident bulk storage will be provided within the basement, accessible by elevator.

Park Place Apartments

A five-story mixed-use building with 43 residential units, one superintendent unit, community facility space, and approximately 26 spaces of underground parking on Vital Brooklyn RFP Site I. The building will include 26 studios, 9 one-bedroom units, and 8 two-bedroom units. In addition, there would be one one-bedroom unit for the superintendent. Park Place will be located in the Crown Heights neighborhood of Brooklyn, just past the border with Prospect Heights.

The Riverview Apartments at Corinth

The project will increase the availability of affordable and supportive housing for individuals and families living in Saratoga County.Amenities include a mail area & secure package room, indoor bike storage, community room with kitchenette, exercise room, & roof top patio. Exterior amenities will include a patio area, bike racks, playground & walking path that connects to the town park. Parking spaces will be provided on a per-apartment basis with additional banked areas for future parking to be constructed if needed by demand.

A six-story, 58-unit affordable / supportive housing residence with program spaces for seniors with incomes up to 60% AMI, pursuant to Affordable Independent Residence for Seniors (AIRS) zoning. The 42,400 square foot building on the corner of Warwick St. & Pitkin Ave. in Brooklyn will include offices for supportive services staff, consultation and medication rooms, a community room, and computer lounge. The project is expected to meet NYSERDA’s Enterprise Green Communities 2015 Criteria with HPD Overlay and will include solar PV panels and green roof.

The residential units are split between four separate buildings connected to a central community hub. Site constraints related to protected wetlands will limit the project to 5 acres of developed area inclusive of the building, parking lots, walkways, landscaping, utility infrastructure and storm water management systems. The 3-story building will include amenities for residents along with case management and care coordination.

Nameoke Avenue Apartments

Nameoke Avenue Apartments will consist of 117 total units of housing; 71 units designated as supportive, service enhanced apartments for formerly homeless individuals with mental health challenges and 45 units targeted for income eligible individuals and families. The existing building, which is in disrepair, will be demolished and an approximately 80,000 square foot, nine story building will be constructed.

 

Amenities will include office and supportive services meeting rooms, a gym, and a community/recreation room for use by building residents.

The development includes standard quality finishes, common areas including laundry, tenant community space, onsite management, surface parking and an outdoor recreation area.

Friendship Lodge

The project will also include the relocation of 12 supported housing units currently subsidized by the NYS Office of Mental Health.

Dominic Hollow Apartments

Development of a 3-story multi-family building providing a total of 60 residential units. Half of the 60 units are designated as Supportive Housing, funded by the Empire State Supportive Housing Initiative (ESSHI), and are intended to house homeless individuals and/or families who struggle with mental illness or substance abuse. The remaining 30 units will be affordable housing, available to individuals and families meeting income criteria.

 

The project will include a community room with kitchen, a computer room, a mail/package room, a gym area, and an outdoor playground for use by all residents. Transitional Services also intends to provide transportation via shuttle bus to planned recreational and social events, as well shopping and other resources.

As part of Governor Cuomo's Vital Brooklyn initiative, NYS Homes & Community Renewal has chosen Herkimer Gardens, a 121 unit mixed-use development with a significant emphasis on Supportive Services, to promote health and wellness for seniors in the Bedford Stuyvesant neighborhood. Federation of Organizations is both the Sponsor and the lead supportive services provider and is a leader in providing housing and daily living assistance to seniors in the NYC Metro area. In addition to housing, Herkimer Gardens will also include on-site urgent care facilities, a wellness center with physical therapy equipment, a food access assistance center, community and recreation space, a computer lounge and a variety of modern amenities designed for senior living and independence.

The proposed infill development consists of seven new buildings spread across three vacant and unused lots, which currently blight the neighborhood. The development will include a Convenience Store and community programming space.
